You are covered 80% up to $300.00 per policy year. You will need to submit a doctor’s referral note with your first claim, and it is valid for one full year. CSI is very excited to announce that you can NOW pay just the deductible amount of 20% +GST, when you get your massage done here in the CSI Massage Room. This service applies only to students under the CSI health plan, all other health plans must pay and submit their claim manually. Paying just the deductible avoids having to pay the full amount and then submit your receipts, which makes getting a massage here even more convenient. So book your appointment today with Josh. Cancellations Clients are required to provide a minimum of 24 hours’ notice to cancel or reschedule an appointment. If less than 24 hours’ notice has been received by CSI, a cancellation fee will be applied to the client’s credit card. No Show A “no show” is defined as a client either not showing up for a scheduled appointment or canceling an appointment with less than 24 hours’ advance notice.   A “no show” will result in the client’s credit card being charged for the full amount of the missed appointment.   After a client “no shows” once for an appointment the client will be required to secure the next appointment with a prepayment. Late for an Appointment Please call if you are going to be late. If no call is received after 15 minutes of your scheduled appointment CSI will assume you are not coming and release the remainder of your time. You will be responsible for full payment of your missed appointment. Doctors Note If a client produces a valid Doctor’s Note for the missed appointment, the appointment may be rescheduled without charge or penalty. She also holds a Bachelor of Sociology from McMaster University. She approaches her work with attention towards her clients' needs and emphasizes client service to ensure they get maximum benefit from their treatment by using a combination of modalities. Joanna looks forward to meeting, treating and guiding you in your journey towards wellness.   Matthew Grubb, RMT Matt is a graduate from Conestoga College in 2003 with a Diploma in Marketing. He received his diploma in massage therapy from Canadian College of Massage and Hydrotherapy when he graduated in 2007.  He has been registered with the College of Massage Therapist of Ontario since 2007.  He has been treating in the public for over two years and has logged more than 2,500 hours of treatment since 2005.
